SURVEYS NEED TO BE SUBMITTED BY MARCH 17, 2008!!

Before you can select the appropriate survey you must first enter a valid login name, which should have been provided to you by the Conference Administrator, Tara Lisle, if not, please call her at 702-598-1710 to obtain one.  This login is your permanent user name for the survey database allowing you to retrieve your prior year's survey each year, update the survey, and resubmit the survey.

Once you enter your login, your survey will appear, review your information that appears, complete the rest of the survey, review your entries, and then submit. Once the survey is submitted you will no longer be able to edit your answers online. If you need to make changes after submission please contact Tara at 702-598-1710.

Please pay close attention to specific instruction on the surveys. These instructions are there to ensure the data is entered into the database properly. Failure to comply with these instructions could corrupt your survey. (All specific instructions are in red text)
